Steps:
1. Secure a transaction slip from the Front Desk Officer.
2. Submit fully filled-up transaction slip to the assigned personnel
3. Wait while the transaction is being processed.
4. Get the certification from the releasing section.

2. Issuance of Division Clearance

Schedule of Availability of Service:
Monday to Friday except Holidays
8:00 AM – 5:00 PM (No Noon Break)

 Who may avail of the Service?
Teaching/Non-Teaching Personnel of the Division of Davao del Sur who  wish to transfer to other Division.

What are the Requirements?
1. Clearance (School/District)
2. Endorsement of Accommodation
3. Service Records
4. Certificate of Last Payment
5. Certification of Last Service/Leave Credits (if any)

Duration:   Under Normal Circumstances – Within 30minutes

Steps:
1. Secure a request/transaction slip from the Front Desk Officer.
2. Submit fully filled-up request form and the required documents to the authorized personnel.
3. Wait while clearance is being processed.
4. Get the clearance from the releasing clerk.

3. Application for Retirement /Separation /Survivorship /Disability

5. Application for Travel Abroad

1. Clearance (School, District, Division) – for 30 days and above
2. Form 6 (Leave Form)
3. Personal Letter of concerned teacher/employee
4. Memorandum to the OIC (for School Heads)
5. Endorsement from the school/district

Duration: Under Normal Circumstances  –Five (5) days

Steps:
1. Submit required documents to the authorized personnel.
2. Wait while the documents are being checked and reviewed by the In-Charge.
3. Get the needed documents after five (5) days from the releasing section.

6. Application for Provident Loan

Duration: Under Normal Circumstances:  15 minutes in all services

Steps:
1. Secure a transaction slip from the Front Desk Officer.
2. Submit fully filed-up transaction slip with other required documents to the authorized personnel or to the cashier for processing.
3. Present 2 valid IDs upon claiming payments/getting the requested documents.

9. Recruitment /Hiring and Appointment of Teacher

Schedule of Availability of Service:
January – April

Who May Avail of the Service?
1. Applicant who has valid certificate of registration/professional license as a teacher from the Professional Regulation Commission (PRC) seeking to be appointed to a Teacher I Position.
2. Qualified applicant who has valid professional license issued by the Philippine Regulation Commission and who meets the requirements of the evaluation /selection process.
3. Bonafide resident of the barangay, municipality, city, or province as evidenced by the Personal Data Sheet and Voter’s ID.
4. List of candidates who obtained an over-all score of 70 points and above based on the criteria provided in the hiring guidelines for Teacher 1 Position.

What are the Requirements?
1. Application Letter
2. CSC Forms 212 (revised 2005) in 2 copies with at least 2×2 ID photo
3. Certified photocopy of PRC certificate of registration/license
4. Certified photocopy of ratings obtained in Licensure Examination for Teachers/Professional Board Exam for Teachers
5. Certified copies of Transcript of Records for Baccalaureate course
6. Copies of service record, performance rating and school clearance for those with teaching experience
7. Certificate of specialized trainings ,if any
8. Certified copy of Voter’s ID
9. NBI Clearance
10. Omnibus certification of authenticity and veracity of all documents submitted.

Steps:
1. Submit a written application letter with the required documents to the nearest elementary or secondary school head.
2. Submit for written and oral interview and conduct demonstration teaching.
3. Take the English Proficiency Test.
4. Wait for the final results of the selection and evaluation conducted.

10. Recruitment/ Hiring and Appointment of Non-Teaching Personnel

Schedule of Availability of Service:
January – April

Who May Avail of the Service?
Any person possessing the qualifications required by the position.

What are the Requirements?
1. Application Letter

Who May Avail of the Service?
Teacher II & III, Education Program Supervisor, Guidance Counselors/Coordinators, and other Teaching and Related Teaching Positions except  School Heads – Principals and Head Teachers, and all Non-Teaching groups.

What are the Requirements?
1. Copies of performance ratings for the last three (3) rating periods.
2. Certification of experiences related to the position to be filled.
3. Certified Xerox copy of outstanding accomplishments
– Outstanding employee award
– Innovation
– Publication/Authorship
– Consultancy/Resource Speaker in Trainings/Seminars
4. Certified Xerox copy of transcript of records with CAV.
5. Certificate of trainings in education attended
6. Certificate of Participation/Recognition as Chairperson/Co-chair in a technical planning committee.

Person/s In-Charge:  HRMO/Division Personnel Selection Board (PSB)/SDS

Duration:

Steps:
1. Watch for published vacant positions in the CSC Bulletin.
2. File application for the position with the accompanying support documents.
3. Wait for further notice on the outcome of preliminary evaluation.
4.Submit for interview and for other assessment before the PSB.

12. Filing of Complaints/Appeals/Motions for Reconsideration

Who May Avail of the Service?
Any person may file an administrative complaint against teaching and non-teaching personnel of the Schools and of the Division Office.

What are the Requirements?
Filing of Complaint
1. Full name and address of the complainant.
2. Full name and address of the person complained of, as well as his position and office in the Department of Education
3. A narration of the relevant and material facts which show the acts or omission as allegedly committed by the person.
4. Certified true copies of documentary evidences and affidavits of his witnesses, if any
5. Certificate of Non-Forum Shopping
6. Complaint shall be under oath
